1cm pedal extenders

I've see cav use da pedals with custom 1cm axles. Shimano currently makes a 4mm option which is great, but a little longer would be optimal. 3 and 4 pedal spacers are a bit much as shimano has not followed looks approach with a longer threaded axle. I've used 2cm adapters which work well, but a bit much in the corners. Any thoughts are where one can get axles that cav had?
