UFC troubles

from what I understand it's not a formal testing regime but rather state by state that they fight in. Some do some don't.
 
As far as I know the UFC (or rather the state athletic commissions where the event takes place in) only tests before/after a fight. So yes, sometimes fighters get caught, but considering there are no out-of-competition tests you really have to be careless to test positive.

They need a drug code and a testing policy that they must sign befor becoming a member wi9thout that you refuse testing simple.

If you sign a antidoping policy then they can test or you get sanction anyway.
