Don't know how much, but I think there's been a consistent pattern of riders who go balls to the wall in Tirreno/Paris Nice underperforming in Sanremo.

I think the 2021 Tirreno is what scares in that regard and that was an outlier in terms of craziness. Ganna doesn't have to do anything close to that.

It's almost as if they designed the parcours so that he might try for GC.

Surely not. They need to dislodge him on the steeper bits and hope his team doesn't drag him back on the shallower bits.
What team would be interested in 1). riding the peloton to shreds in 1,5 kilometers, and subsequently 2). ride it all the way to the line? I think there are no such teams.

This is even in the hypothetical they are able to drop Milan like a stone, who, time and time again, has shown he can climb rather well despite his enormous frame.
