It's been a while like that. They calculate only the time up the climb.

KOM points in ITT climbs have been since forever. KOM points awarded on the time up the climb, rather than time from the start is a recent gimmick.

Yes. There is 10-rider group 15 minutes from the leaders. Gruppetto is 20 minutes behind leaders.
