He can follow a team mate's attack. In a sense he's part of that attack. He needs the cramp to get some non-crampy legs, attack and follow that attack. I would say Caruso is now the favourite. He needs everything to go his way to podium. This Giro most of the top contenders imploded on one climb or the other and he has taken advantage by being the most consistent and not wasting energy. But there is not much he can do in this Giro. He's already lost three major teammates. Even if he takes pink, he and his team will be hard pressed to hold it
