Fatigue is a factor in endurance sport if you actually have stages long and difficult enough to make endurance a factor. Who knew?

I don't know what they've done recently, but back in 2011 he lost like 25 points.

You lose the number of points that would be available for winning the stage in question, such that you are penalised more heavily for missing the time cut in a flat stage than a mountain stage. Only way to make it less would be in an ITT, but obviously you aren't going to be in a group big enough to not be kicked out in an ITT.
