Very unfortunate, but completely her own fault. Quite amateurish, actually.
Well, if it wasn't a flat or any other mechanical, it did seem like it was simply bad steering, and there are hardly any curves in the parcours.

The wobbling is typical for loosing control in the aero position (so it doesn't look like a flat to me, but looking at the replay, I just don't know. It could as well have been a badly glued tubular or whatever tire system she is using). Besides Dygert's own lack of concentration / bike handling (keeping in the aero position), the car behind should have warned her for the curve.
