He was not in the middle. The crash was near the front, but most of the peleton managed to avoid it, cause the road was wide enough.

Yes I noticed this from the front shot and was surprised keenan missed it on the commentary here, orange shirt went down near the front probably just behind the team (katusha was it) running the pace at the time.

I don't think he did, at least not right away. I thought the reason for Caisse d'Epargne not featuring on the climb at all was that some of them waited for him. If they did, then he obviously kept got back on his bike and they thought they should still stick with him as their best bet for the GC, because of his time trial skills. Just my speculation though.
